Sanitas Mundi - Grounds for contract ( health insurance )

1. This contract has been entered into on the basis of the statements made by the Policyholder and Member on the Insurance Application and Medical Questionnaire concerning the Member's state of health, occupation and preferred sports. The aforesaid statements constitute the basis for acceptance of the risk covered hereby and form an integral part hereof.

2. Should the Policyholder fraudulently omit any circumstance known thereto which could affect risk appraisal, the Insurer is entitled to cancel the contract within thirty days after the date on which the aforesaid omission came to its knowledge.

3. The Policy shall be indisputable one (1) year after the date on which this contract comes into effect, barring fraud on the part of the Policyholder or Member.

4. In the event that the Member's date of birth is stated inaccurately, the Insurer may challenge the validity of the contract only if the Member's real age is over the admission age limit established thereby when the contract comes into effect.

Otherwise, if, as a result of the inaccurate statement of age, the premium is computed to be lower than it would have been the Member shall be bound to pay the Insurer the difference between the amounts actually paid in premiums and the amounts that should have been paid on the basis of the Member's real age.

If, on the contrary, the premium is computed to be higher than it should have been, the Insurer shall be bound to refund the excess premium paid interest-free.
